(א) הַמִּתְפַּלֵּל צָרִיךְ שֶׁיְּכַוֵּן בְּכָל הַבְּרָכוֹת, וְאִם אֵינוֹ יָכוֹל לְכַוֵּן בְּכֻלָּם, לְפָחוֹת יְכַוֵּן בְּאָבוֹת; אִם לֹא כִּוֵּן בְּאָבוֹת, אַף עַל פִּי שֶׁכִּוֵּן בְּכָל הַשְּׁאָר, יַחֲזֹר וְיִתְפַּלֵּל. הַגָּה: וְהָאִדָּנָא אֵין חוֹזְרִין בִּשְׁבִיל חֶסְרוֹן כַּוָּנָה, שֶׁאַף בַּחֲזָרָה קָרוֹב הוּא שֶׁלֹּא יְכַוֵּן, אִם כֵּן לָמָּה יַחֲזוֹר (טוּר).
(1) One who prays needs to have concentration on all the blessings, but if he is not able to concentrate on all of them, he should at least concentrate on the blessing of the forefathers (the first blessing of Shemonah Esrei), but if he doesn't have concentration while saying the blessings of the forefathers, even if he has concentration on the rest [of the blessings], he needs to return [to the beginning of Shemonah] and pray. Gloss: Nowadays we do not return because there is a loss of concentration, and even when he returns [to the beginning of Shemonah Esrei] he will not have concentration; if so, why should he return (Tur)?
Simeon HaPakuli arranged the eighteen benedictions in order in the presence of Rabban Gamaliel at Jabneh. Rabban Gamaliel asked the Sages, "Is there anyone who knows how to word the benediction relating to the Minim (the heretics)?" Samuel the Small stood up and worded it. In the following year he had forgotten it,
והשקיף בה שתים ושלש שעות ולא העלוהו אמאי לא העלוהו והאמר רב יהודה אמר רב טעה בכל הברכות כלן אין מעלין אותו בברכת הצדוקים מעלין אותו חיישינן שמא מין הוא
and reflected for two or three hours ; but they did not discharge him. Why did they not discharge him ? For lo, Rab Judah said in the name of Rab : If [the Precentor] errs in all the [eighteen] benedictions we do not discharge him ; but [if he errs] in the benediction relating to the Minim we do discharge him, for fear that he is a Min (a heretic) !
